  Dr Mark Muller:
  No surcharge for injuries with a registered ACC claim.​
  
  Dr Scott Annett:
  Initial consultations - $130 to $175 with a registered ACC claim.
  Follow up consultations - $80 to $125 with a registered ACC claim.
  
  Non-guided Corticosteroid injection: $150 (a follow up appointment is also required 4 weeks later, at follow up charge).
  Ultrasound guided Corticosteroid injection $255 (a follow up appointment is also required 4 weeks later, at follow up charge).
